New patient packet

Please make sure you complete the new patient packet prior to your first visit with IPC. New patients can be seen within one week.

Patient referral form

If you are seeking a referral from your primary care provider, they will need to fill out the patient referral form and fax it to 618.465.7176. They will be able to note the preferred IPC location on the referral.
